ABOUT THE PROJECT

Polish Mother's Memorial Hospital – Research Institute, together with Partners, invites all interested people diagnosed with heart failure to participate in the project.

Heart failure is not a single disease, but a clinical syndrome characterized by certain symptoms.

The essence of heart failure is that the heart is unable to pump enough blood throughout the body to supply organs with oxygen and other blood components.

Any cardiac disease, especially untreated, can lead to heart failure!

• hypertension

• dyslipidemia, or abnormal cholesterol

• valvular defects

• coronary artery disease (including a history of myocardial infarction)

• constant monitoring of parameters,

• permanent access to a primary care physician and cardiologist,

• increased quality of life,

• comprehensive care for patients with heart failure,

• real systemic support for seriously ill patients,

• easier access to educational materials
